| Sick Bank |
8 days per year (pro-rated from date of hire) , with non-used sick time rolled over up to a maximum of 16 sick days at no cost to you. |

Basic Life Insurance of 2 times your annual pay. |
| Life Insurance, AD&D Voluntary Coverage |
Employee coverage up to 7 times salary (max. $300,000) with spouse (max. of $150,000) and child coverage available. Employee rates are based on age and elected amount. Child coverage rate is $2 a month ($10,000) . |
| Short Term Disability |
90% for the first 30 days, then 75% up to the maximum of 180 days at no cost to you. |
| Long Term Disability |
66.67% of base salary, maximum of $15,000 per month. Unlimited lifetime maximum. |

Free coverage up to $100,000 while traveling on Midwest ISO business. |
